1. Introduction

GP Triage Ltd is committed to preventing modern slavery and human trafficking in all its forms. This statement is made voluntarily in accordance with the spirit of Section 54 of the Modern Slavery Act 2015. Although GP Triage Ltd does not meet the annual turnover threshold that makes a transparency statement a statutory requirement, we believe it is right to publish this statement as part of our commitment to ethical business practice and responsible supply chain management.

This statement covers the financial year ending 31st March 2026 and will be reviewed and updated annually.

2. Our Organisation and Supply Chain

GP Triage Ltd (Company No. 14797566) is a health technology company providing an autonomous AI-powered triage and appointment booking platform for NHS primary care. We are a fully remote organisation incorporated in England and Wales, with our registered office at 7 St. James Place, Birmingham, B7 4JE.

Our workforce consists of:

  • Two directors based in the United Kingdom
  • Two members of staff based in the United Kingdom
  • One part-time contractor based in the United Kingdom
  • Development services provided by Cloud Surge, whose team of developers is based in Pakistan, engaged under a formal service arrangement
  • Three GP Ambassadors operating as referral partners under written agreements

Our primary supply chain relationships include Microsoft Azure (cloud infrastructure), Infermedica (AI clinical decision support), Hero Health (appointment booking API), and Qyro (regulatory and data protection advisory services). All supplier relationships are governed by written contracts.

3. Our Policies

GP Triage Ltd maintains a Modern Slavery Policy which sets out our zero-tolerance approach to slavery and human trafficking. This policy applies to all personnel, contractors, and supplier relationships. It is reviewed annually and approved by the Chief Executive Officer.

Our wider policy framework, maintained under our ISO 27001 Information Security Management System, includes a Supplier Information Security Policy, a Supplier Onboarding SOP, a Code of Conduct, and an Equality, Diversity and Inclusion Policy, all of which reinforce our commitment to ethical conduct throughout our operations.

4. Due Diligence

GP Triage Ltd takes the following steps to identify and manage modern slavery risk across its operations and supply chain:

  • All suppliers are onboarded through a formal process that includes review of their policies and governance arrangements
  • Supplier contracts include obligations relating to ethical conduct and compliance with applicable law
  • Cloud Surge, our Pakistan-based development services partner, is required to confirm that all personnel working on GP Triage systems hold clear written contracts in a language they understand
  • No cash payments are made to any workers. All remuneration is made through formal payroll or invoiced payment processes
  • Any concerns raised by personnel or suppliers regarding suspected modern slavery are escalated directly to the CEO for immediate action

5. Risk Assessment

We have assessed our modern slavery risk as low, for the following reasons:

  • Our UK-based workforce is small, directly employed or contracted, and paid through formal processes
  • Our primary supply chain partners are established technology organisations operating in regulated environments
  • Our Pakistan-based development services are provided through Cloud Surge, a formal business entity under contract, not through informal labour arrangements
  • The nature of our service is digital and does not involve manufacturing, logistics, or sectors traditionally associated with elevated modern slavery risk

We recognise that Pakistan is identified as a higher-risk geography in some modern slavery indices. We mitigate this risk through formal contractual arrangements with Cloud Surge and by requiring confirmation of appropriate employment practices for all developers engaged on GP Triage work.

6. Training and Awareness

All GP Triage personnel are made aware of this statement and of the risks of modern slavery as part of their onboarding. The Modern Slavery Policy is included in the GP Triage New Starter Pack and is available to all staff at all times.

The Chief Executive Officer retains responsibility for ensuring that modern slavery considerations are understood and acted upon across the business.

7. Measuring Effectiveness

GP Triage Ltd measures the effectiveness of its steps to address modern slavery through the following indicators:

  • Zero incidents of modern slavery identified or reported in the reporting period
  • All supplier contracts reviewed and confirmed to include ethical conduct obligations
  • Modern Slavery Policy reviewed and approved by CEO annually
  • All new personnel confirmed as having read the Modern Slavery Policy at onboarding

8. Approval

This statement has been approved by the board of directors of GP Triage Ltd and will be reviewed annually.
